After you select a date and time from the calendar, a link to your Intake Form will be sent to your email. It takes 10-20 minutes to fill it out online, which allows for more treatment time in your session.  

For smooth scheduling and best care; it is recommended to book 1-2 Follow-Ups ~1-2 weeks apart, which can later be changed if not required or a different schedule is deemed more beneficial to your treatment. 

Upon full payment, a receipt will be issued which you can submit to your insurance provider for reimbursement if applicable

Please bring loose shorts and a baggy t-shirt if possible.

Assessment + Treatment

This Session includes: 

A check-in on your progress
Manual osteopathic therapy as needed
Tailored take home care unique to your situation 

Please make sure you have attended an "Assessment + Treatment" at least 1x in the last year before booking a follow-Up. If it has been more than a year, then we need to recollect your updated health history and give extra time to go over your case, which will be accounted for by booking another "Assessment + Treatment" session.
