For Manual Osteopathic Therapy Appointments:

After you select a date and time from the calendar, a link to your health history form will be sent to your email. It takes 10-20 minutes to fill it out online, which allows for more hands-on treatment time in your session! 

• Upon full payment, a receipt will be issued which you can submit to your insurance provider for reimbursement if applicable.
• Please note the 24 hour cancellation policy.
• Please bring loose shorts and a baggy t-shirt if possible
